Formal translation:
```slovak
Chcel by si byť mojou valentýnskou?
```
Casual translation:
```slovak
Chceš byť mojou valentýnskou?
```
Explanation:
The formal translation uses the conditional form "Chcel by si" which is more polite and respectful. The casual translation uses the simple present tense "Chceš" which is more informal and friendly. The choice of verb form and word order also contributes to the formality or casualness of the translations.

We wouldn’t really say if sb. wants to be our valentine (at least I've never heard it), I would say we are more inclined to say something like: Would you like to go out with me on Valentine's day? (Chcel/a* by si ísť so mnou von na Valentína?). But maybe some couples ask the question you typed - Budeš mojou Valentínkou? (it really sounds weird to me). And I bet there are some people who are very romantic and like to do it their own way.
People (mainly teenagers) always ask each other : Who are you going out with on Valentine's day?
btw the old generation here doesn't really celebrate this day. I would say it's still something "new" here...
*depends on the gender (chcel - a woman asking man, chcela - a man asking woman)
